HOW DO I.... REGISTER
WITH THE PRACTICE?
Before we can accept you onto our list you must complete our application form to the best of your ability. You will be given a urine sample bottle and an appointment where we will take some basic measurements: BMI, blood pressure and complete the computerised registration process. Please remember to bring your sample with you. If you have a computerised list of medication from your previous practice please bring it with you or you could bring the empty boxes; this is to ensure we get all the correct information. |
